about you to nonaffiliated third parties, you may opt out of those
disclosures, that is, you may direct us not to make those disclosures (other
than disclosures permitted by law). To opt out, complete the blanks below and

Once your signed statement is received, we have a reasonable amount of time to
stop the information sharing. To revoke your opt out, send your signed
statement saying you will permit us to share the information indicated.
